II / INSMART
Examining the scope underwriting fees
If covered by insurance
– Confirm BLVP agree within 15 minutes.
– Specify the level of compensation for feedback by phone, fax or email.
1. Notification of guarantee fees for patients.
2. Implement medical treatment under the regulations of the Ministry of Health
Detecting violations insurance card use.
3. Complete Profile BLVP:
– Administrative Documents: Original
– Medical documents: the original or a copy certified true copy.
– Proof of payment: the original, the date of Receipt / Invoice Finance
4. Within 15 days after the end of clinic patients, dental treatment all dossiers sent guaranteeing payment for hospital fees InSmart
– Signed by pens, full name on the “Request for guarantee fees and claim”.
– Check the cost of healthcare was used, written with ink pen, full name on receipts / invoices Finance / detailed list.
– Payment of costs and treatment outside the scope of the guarantee fee
If not covered by the guarantee fees:
– Confirm refuse underwriting fees.
– Specify the reasons for refusal to guarantee feedback by phone, fax or email.
– Notify the insured.
Payment of care costs, treatment

– Check the administrative procedures and medical evaluation khoa- announced additional documents (if any)
– Reimbursement of expenses BLVP within 30 days after receiving a complete dossier.
